Payment terms for full settlement are due within 30 days of the statement date. Unsettled balances beyond 30 days will accrue a monthly interest of 2% (24% annualized).

To ensure uninterrupted service, any unpaid balances extending beyond 30 days will necessitate prepayment for any new work requested until the account is in good standing.

Unsettled balances exceeding 45 days will prompt a suspension of all ongoing work until the account is brought to a favorable standing. Failure to settle the balance within 15 days of the initial 45-day notification will result in the referral of the outstanding amount to an external collection agency.

We gladly accept check payments should you prefer this method. It is important to note that in the event of a returned check due to Non-Sufficient Funds (NSF), D&J Dental Lab Inc reserves the right to charge any associated fees incurred from financial institutions.

We commit to promptly informing you of any outstanding balances and addressing all billing concerns.
Payment Policy

Accounts in good standing have the option to accumulate invoices over a 90-day period, after which they can be consolidated into monthly statements issued on the first day of the following month.

Please ensure you take note of the following: Payment is due by the 11th, and a detailed statement will be sent to you by mail.
Remake & Warranty Policy

When you submit a case to D&J Dental Lab Inc, you are indicating your acceptance of the terms of the Remake & Warranty Policy.

Unfortunately, once custom-made dental appliances have been fabricated, the cost is non-refundable.

D&J Dental Lab Inc. may issue a credit directly to a customer’s account if the cases do not meet the lab's quality, function, and aesthetic standards. Any credit balance attributed to an account held with D & J Dental Lab Inc is required to be utilized or applied towards lab services within 60 days from the date of issuance, failing to do this will expire.

Remake Policy: If you send in an eligible remake within 60 days of the invoice date, it will be done at no charge. In order to be considered for credit, you must return all original case materials, including models and impressions, alongside the dental restoration.

If a remake is necessary due to any of the following reasons, new charges will apply:

Any change in shade or product differs from the initial request, including but not limited to material type, shade, design, and so on.

The lab had doubts about the impressions, margin, or bite and was instructed to finalize the case.

The lab highly recommends a try-in to ensure the best results. However, the customer insisted on receiving the completed case instead.

Reduction of abutment was necessary because of undercuts/clearance and/or the teeth underwent re-preparation.

The item will be remade with a new impression that was sent.

The lab script did not mention the patient's allergy before creating it.

The treatment plan was communicated verbally instead of being properly submitted with a new laboratory prescription, which is a crucial step for effective implementation.

The patient or office canceling a treatment plan does not justify a return or credit, and the laboratory is not associated with patient-to-provider plans.

The warranty period is one year from the date of delivery as stated on the invoice.

This warranty serves as the exclusive warranty, superseding all other warranties, whether explicit or implicit, and is not subject to alteration by any representative, employee, agent, or distributor of D&J Dental Lab Inc.
